An original birth certificate (or other proof of identification i.e., Passport, Federal ID, Government ID), social security card (or two other proofs of social i.e., voter registration card, w-2 or w-4, Military issued ID), two proofs of address (lease, rental agreement, gas bill, electric bill, water bill, bank statement) must be presented. Only persons with expired driver's license over 1 year and less than 5 years, persons with their license revoked or suspended over 1 year due to DUI, or first time applicants for a driver's license will need to take the written test.
